Unbiased estimation in seamless phase II/III trials with unequal treatment effect variances and hypothesis‐driven selection rules

چکیده
Seamless phase II/III clinical trials offer an efficient way to select an experimental treatment and perform confirmatory analysis within a single trial. However, combining the data from both stages in the final analysis can induce bias into the estimates of treatment effects. Methods for bias adjustment developed thus far have made restrictive assumptions about the design and selection rules followed. In order to address these shortcomings, we apply recent methodological advances to derive the uniformly minimum variance conditionally unbiased estimator for two-stage seamless phase II/III trials. Our framework allows for the precision of the treatment arm estimates to take arbitrary values, can be utilised for all treatments that are taken forward to phase III and is applicable when the decision to select or drop treatment arms is driven by a multiplicity-adjusted hypothesis testing procedure. © 2016 The Authors. Correcting for selection bias in two-stage trials with multiple correlated outcomes: application to seamless phase ii/iii clinical trials
The problem of selection bias has long been recognised in the analysis of two-stage trials, where promising candidates are selected in stage 1 for confirmatory analysis in stage 2. To efficiently correct for bias, uniformly minimum variance conditionally unbiased estimators (UMVCUEs) have been proposed for a wide variety of trial settings, but where the populations are assumed to be independent...
متن کاملConditionally unbiased estimation in phase II/III clinical trials with early stopping for futility
Seamless phase II/III clinical trials combine traditional phases II and III into a single trial that is conducted in two stages, with stage 1 used to answer phase II objectives such as treatment selection and stage 2 used for the confirmatory analysis, which is a phase III objective. Although seamless phase II/III clinical trials are efficient because the confirmatory analysis includes phase II...
متن کاملEstimation bias in survival data within clinical trials that use adaptive seamless designs
Adaptive designs are utilised in late phase clinical trials due to their efficiency in answering multiple clinical questions through a single trial. For example, seamless phase II/III clinical trials are used to answer phase II and III objectives via a single trial with two stages, where stage 1 and 2 represent phase II and III components respectively. At the end of stage1, an interim analysis ...
متن کاملOptimizing the data combination rule for seamless phase II/III clinical trials
We consider seamless phase II/III clinical trials that compare K treatments with a common control in phase II then test the most promising treatment against control in phase III. The final hypothesis test for the selected treatment can use data from both phases, subject to controlling the familywise type I error rate. We show that the choice of method for conducting the final hypothesis test ha...

BACKGROUND TNK-S2B, an innovative, randomized, seamless phase II/III trial of tenecteplase versus rt-PA for acute ischemic stroke, terminated for slow enrollment before regulatory approval of use of phase II patients in phase III. PURPOSE (1) To review the trial design and comprehensive type I error rate simulations and (2) to discuss issues raised during regulatory review, to facilitate futu...
